A Hot, Swollen or Painful Calf
Or sudden shortness of breath or chest pain. Call 911 first, then us. These are the signs of a clot.¹
You walk out the same day. Then what? Here is what the first day, the first week and the first months look like after the three procedures we do most, what helps, and the few signs that mean you call us right away.
General guidance · Your surgeon’s written instructions come first
In short
You walk out the same day. Desk work in a few days after liposuction, about a week after FlexTuck, and one to two weeks after a BBL with a cushion. Light exercise at two to four weeks, hard exercise around six. The shape is close to final at three months and settles until about six. Your surgeon’s written instructions set your dates.
Side by side
| Awake liposuction and FlexTuck | Brazilian butt lift | |
|---|---|---|
| Surgery day | You walk out awake, in your compression garment, with someone to drive you home. Expect clear fluid from the small incisions for a day. That is the tumescent solution, not blood. | The same. Plus: lie on your front or side tonight. Nothing presses on the buttocks. |
| Days 1 to 3 | Soreness like a hard workout, swelling, some bruising. Short walks every hour you are awake. Garment stays on except to shower. | The same, with no sitting directly on the buttocks. A cushion under the thighs if you must sit for a few minutes. |
| Week 1 | Most people are back at desk work within days after liposuction, and in about a week after FlexTuck. First follow-up. Gentle lymphatic massage only if your surgeon recommends it. | Back at a desk in a week or two, on the cushion. Still no long stretches sitting or lying on the buttocks. |
| Weeks 2 to 4 | Swelling eases. Light exercise returns. Garment still on most of the day. Numb or firm patches are normal and soften with time. | Sitting gets easier with the cushion. Walking only. No squats, no cycling yet. |
| Weeks 4 to 8 | Hard exercise and heavy lifting return as your surgeon clears them. Garment hours taper. | Normal sitting returns gradually. The fat that has survived is now yours to keep. |
| Month 3 and after | The shape is close to final. Small firm areas keep softening until about month six. | We call the result at a few months, not a couple of weeks. The shape at two weeks is nowhere near the finished one. |

What helps most
Four habits that do most of the work.
Short walks keep blood moving in your legs. Lying still for hours is what raises the risk of a clot after any surgery.¹ ²
It supports the skin and controls swelling. Off to shower, then straight back on.
Protein, water and sleep. No alcohol while you take any pain medicine.
Small dressings, changed as instructed. Fluid on the first day is expected. Redness that spreads is not.
Warning signs
Or sudden shortness of breath or chest pain. Call 911 first, then us. These are the signs of a clot.¹
Or spreading redness, heat, or a bad smell at an incision.
Sudden one-sided swelling can be fluid or blood collecting. We want to see it the same day.
Call the number on your written instructions. In an emergency, call 911 first. There is also a published route for any concern.
The evidence
No grogginess or nausea from a general anesthetic, no hours in a recovery bay. In 15,336 awake tumescent cases there were no hospital stays.³
Tumescent fluid shrinks the small vessels. Blood loss is minimal, so you are not wiped out on day one.⁴
You walked out of the room. That head start matters for clots and for how you feel tonight.¹
FAQ
Desk work within days after awake liposuction, about a week after FlexTuck, and a week or two after a BBL with a cushion. Physical jobs wait longer. Your surgeon gives you a date.
Walking from day one. Light exercise after two to four weeks. Hard exercise and heavy lifting when your surgeon clears you, usually around six weeks.
Around the clock at first, off only to shower, then tapering over the following weeks as your surgeon directs.
Not directly on the buttocks for the first two weeks or so, then on a cushion under the thighs. Normal sitting returns gradually over the following weeks. Your surgeon sets the dates.
The evidence is mixed.⁶ It is low-risk when gentle and done by someone trained. Ask your surgeon, and read our aftercare page.
The shape is close at three months and settles until about six. The result at two weeks is swelling, not the result.
Soreness, swelling, bruising, numb or firm patches and clear fluid on day one are normal. A hot painful calf, shortness of breath, fever, spreading redness or sudden one-sided swelling are not. Call.
