Liposuction at a High BMI
What Is Possible, and How It Is Kept Safe

A high BMI does not rule out liposuction. At Goals, awake liposuction is offered up to a BMI of 65, after a medical review. Here is how it is planned and kept safe.

In short

The Short Answer

A high BMI does not rule out liposuction, whether you are overweight or living with obesity. No law in the states where Goals operates sets a BMI limit. Some risks rise with weight, much of it from general anesthesia.¹ That is why a BMI near 35 is a common cutoff. Liposuction at Goals is done awake under local anesthesia, in planned sessions, after checks before surgery. It is offered up to a BMI of 65, for small areas at the higher end. It reshapes an area. It is not a way to lose weight.² Your surgeon confirms what is right for you.

What Can Be Done at Each BMI?

At Goals, your BMI decides which plan is offered. Your health and your assessment decide the rest.

A BMI scale from 20 to 70. Lipo 360 up to 31, Full Front or Full Back from 31 to 38, small areas only from 38 to 65, and a weight loss program first above 65. Awake FlexSculpt is offered up to 65.

Slide the dot to your BMI to see which plan fits.

  1. 31 and UnderLipo 360Seven areas around your middle in one visit, also offered with a Brazilian Butt Lift.
  2. 31 to 38Full Front or Full BackFive areas on one side of the body. Fat transfer is offered up to a BMI of 38.
  3. 38 to 65Small Areas OnlyAwake liposuction of small areas, without fat transfer, if that is what you are interested in.
  4. Over 65Weight Loss FirstOur physician-guided weight loss program can be the first step.
Your surgeon confirms your plan after a medical review. BMI is one number. Your health, where your fat sits and your skin matter too.

The reasons

Why Do Surgeons Stop at a BMI of 35?

Most of the reasons come from general anesthesia and from doing too much in one day. Here is each one, and how it is handled.

Blood Clots

A higher weight is one of the things that raises the chance of a clot in the leg or lung after surgery. A clot risk score before surgery decides what protection you get.³ ⁴

Breathing Under General Anesthesia

Asleep, extra weight and sleep apnea make breathing harder to protect. Awake liposuction does not put you to sleep, so this risk is not part of it.⁵

Fluid Shifts

Liposuction moves a lot of fluid in and out of the body. Planned, smaller sessions keep each shift small.⁶ ⁷

Wound Healing and Infection

In a study of 127,961 cosmetic patients, infections were about 2.7 times as common at a BMI of 30 or more.¹ Liposuction uses small incisions, checked at every follow-up.

Compare

Awake vs Asleep Liposuction at a High BMI

Awake (Local Anesthesia)Asleep (General Anesthesia)
How You Are NumbedDilute numbing fluid goes into the fat. You stay awake and can talk.Medicine puts you fully to sleep.
Your BreathingYou breathe on your own the whole time. The anesthesia team supports your breathing. Extra weight and sleep apnea need extra planning.⁵
Checking the ShapeYou can shift position, so the surgeon can judge the contour as you move. You lie still, and the team turns you.
Going HomeThe same day, with someone to drive you.Often the same day. Large sessions call for an overnight stay.⁸
If You Take a GLP-1No sedation, so the slow-stomach concern matters less. Your surgeon still reviews it.Anesthesia societies advise a plan for the medicine before you are put to sleep.⁹
What Stays the SameThe risks of surgery: bleeding, infection, uneven contour and, rarely, clots. The same risks of surgery, plus those of general anesthesia.

Both are used safely every day. At a higher BMI, taking general anesthesia out removes one layer of risk.

Awake or asleep

Is Awake Liposuction Safer at a High BMI?

Dilute numbing fluid goes into the fat first, and you stay awake. The breathing risks of general anesthesia are not part of it. Awake does not mean risk free: bleeding, infection, uneven contour and, rarely, clots can still happen.

Worked example

How Much Fat Can Be Removed at a High BMI?

No law in the states where Goals operates caps the volume. Guidance draws the line at 5,000 cc of total aspirate, fat plus fluid, in one session.⁸ ¹³ One registry study also found problems became more likely above about 100 mL per point of BMI.¹⁴ Here is what that works out to.

Bar chart: about 3,000 mL at BMI 30, 4,000 at 40, 5,000 at 50 and 6,000 at 60. A dashed line at 5,000 cc; the BMI 60 bar goes over it.

Tap a bar to compare it with the 5,000 cc line.

An illustration of one study, not a plan. Volumes are total aspirate: fat plus fluid. Above a BMI of about 50, the 5,000 cc line, not the BMI, sets the limit. At Goals, above a BMI of 38, liposuction treats small areas only, so sessions stay well below these numbers. Your surgeon sets your session size.

What Does a Staged Liposuction Plan Look Like?

When one session is not enough, the plan is split. You know the whole plan before the first.

  1. 01

    One Plan, in Writing

At your consultation you get the number of sessions and the price for all of them, before you commit to the first.

  2. 02

    Session One

The first areas are treated, awake, and kept within a safe size for one day. You go home the same day.

  3. 03

    Heal and Check

Your surgeon checks your healing before the next date is set.

  4. 04

    The Next Session

The next areas are treated the same way: awake, planned, home the same day.

  5. 05

    The Final Shape

Swelling goes down over weeks after each session. The final shape shows over months.

Before surgery

What Tests Do You Need Before Lipo?

At a higher BMI, the checks matter more. Here they are, in order.

  1. 01

    A Full Medical History

Your health, past surgeries, clots in you or your family, and every medicine and supplement you take.

  2. 02

    Blood Tests and Clearance

Lab work and a medical clearance before any surgery date is set. Weight is one of the things that raises risk in day surgery.¹⁵

  3. 03

    A Clot Risk Score

A short checklist that scores your clot risk and decides what protection you need.⁴

  4. 04

    Your Medicines, Including GLP-1

Tell us if you take a GLP-1 medicine. Follow the plan your surgeon and prescriber agree on.⁹

  5. 05

    A Steady Weight

A weight that has held for a few months gives your surgeon a shape to plan around.¹⁶

Results

What Results Are Realistic at a High BMI?

Shape Changes More Than the Scale

Liposuction removes fat from a place. In a study of fat removal from the belly, weight and health markers barely moved.²

Loose Skin Is Possible

How well skin shrinks back depends on your age, your skin and how much fat comes out. Your surgeon tells you before, not after.

Results Take Months

Swelling goes down over weeks. The final shape shows over months.

Keeping It Is Up to You

Fat removed from an area does not come back there. Weight gained later goes somewhere else.

Individual results vary. Your surgeon will tell you what is realistic for your body.

FAQ

Questions About Liposuction and BMI

No law in the states where Goals operates sets one. Each surgeon and facility sets its own, based on how the surgery is done. Your surgeon decides what is safe for you.

There is no single number. At Goals, Lipo 360 of the whole middle is for a BMI of 31 and under. The Full Front or Full Back package is for 31 to 38. Small areas are offered up to 65. Your surgeon confirms after a medical review.

Yes, for many people, after a medical review. Above a BMI of 38, awake liposuction at Goals treats small areas, up to a BMI of 65. The page on surgery at a higher BMI explains more.

Not the full Lipo 360 of the whole middle, which is for a BMI of 31 and under. From 31 to 38, the Full Front or Full Back package treats five areas on one side of the body.

Some risks rise with weight, such as clots and wound problems. Being awake, planned sessions and checks before surgery are how they are kept low. Every operation carries some risk.

Very little. Liposuction changes your shape far more than your weight. It is not a weight-loss operation.

Not necessarily. Many people have liposuction at a higher BMI. If a lot of weight is still coming off, waiting can give a better result. Your surgeon will tell you honestly.

Tell your surgeon you take it. Do not stop or change it on your own. Your surgeon and your prescriber agree on the plan.
