Scope
A concern is anything about your care at Goals that did not go the way it should: a result you are worried about, pain or healing that does not feel right, how you were spoken to, a billing or scheduling problem, or something that happened in the clinic. You do not need to be sure it is serious, and you do not need to have spoken to anyone first. This page is the direct route to the team that resolves it.
How it works
Three steps, with one person responsible for yours.
Use the form below, or call or email us. Tell us which clinic, when, and what happened.
Your submission goes to the Goals team, not to a sales queue. A team member is assigned to your case and reviews it with the clinic that treated you.
That person contacts you to go through the concern, the options and what happens next, and stays your point of contact until it is resolved.
Concern form
It takes a few minutes. Include as much as you can; photos and documents can follow by email once we are in touch.
Your submission goes to the Goals team, not to a sales queue, and is handled as confidential patient information.

FAQ

The Goals team responsible for patient concerns, and the clinical lead at the clinic that treated you. It does not go to a sales queue and does not create a sales record. It is handled as confidential patient information.

No. Follow-up appointments, any revision your surgeon has agreed to, and your access to the clinic continue as before. Raising a concern is part of normal care, not a step against it.
No. You can come straight here. If you have already raised it at the clinic, say so and tell us who you spoke to, and the person assigned picks it up from there.
You see a confirmation page immediately. A team member is assigned, reviews your case with the clinic that treated you, and contacts you by the method you chose to go through the concern and the next steps.
This route is for your own care at Goals. If something you read made you hesitate, bring it to a free consultation and ask. We would rather answer it than leave it unanswered.
