What a lip lift is

A subnasal or bullhorn lip lift removes a precisely measured strip of skin from directly beneath the nose, following the curve of the nostrils and the base of the columella. When the wound is closed, the upper lip is drawn upward and rolls slightly outward, so more of the pink vermilion is visible and the upper teeth show at rest. It changes proportion rather than volume: the lip is not made bigger, it is made to reveal what was already there. The amount of skin removed is small — typically a few millimetres — and it is measured against your own philtral length rather than a standard.

Lip lift or lip filler

These solve different problems and the distinction is worth getting right. Filler adds volume to a lip that is thin. It works well when the philtrum — the distance from nose to lip — is a normal length. When the philtrum is long, adding filler pushes the lip forward and downward instead of upward, which produces the heavy, projecting look people are trying to avoid, and doing it repeatedly can lengthen the lip further by stretching the skin. If you have been having filler and feel it is no longer giving you what it used to, a long philtrum is very often the reason, and the lift is the operation that addresses it.

Who makes a good candidate

Good candidates have a long upper lip — often over about two centimetres from the base of the nose to the top of the lip, though this is judged against the rest of the face rather than by a number alone — little or no tooth show when at rest, and a desire for a permanent change. Older patients whose lip has lengthened with age and younger patients with a naturally long philtrum are both common. Patients with very short philtrums, prominent nostril bases, or a history of poor scarring need a careful conversation. Anyone unwilling to accept a scar at the base of the nose should not have this operation.

How the operation is done

Under local anesthesia, with or without sedation, in an accredited facility, taking under an hour. Measurements are taken with you sitting upright and your face at rest, since lip position changes with expression and posture. The incision is drawn hugging the base of the nose and the nostril sills — the bullhorn shape that gives the technique its name — so that the scar hides in the natural shadow and crease there. The measured strip of skin is removed, the deeper tissue is closed to take the tension, and the skin is closed with very fine sutures. Precision at the closure is what determines whether the scar is invisible or not.

Recovery, honestly

Swelling is significant for the first three to five days and the lip will look considerably larger and more everted than the final result — nearly every patient panics at this stage and nearly every one is fine. Sutures come out at five to seven days. Most people are back at work within a week, and presentable with a little makeup sooner. Smiling and eating feel tight for a fortnight. The scar goes pink at around three weeks, which is when patients notice it most, and then fades over six to twelve months to a thin line in the shadow of the nose. Silicone gel and sun protection help materially.

The scar, honestly

The whole procedure lives or dies on the scar. Placed exactly at the base of the nose, following the natural curve, and closed without tension, it becomes very difficult to see within a year — most people have to be told where it is. Placed a few millimetres too low, on flat skin rather than in the crease, it stays visible permanently. This is why the operation should be done by someone who does it regularly, and why photographs at a year, taken from directly in front and from below, are the ones to ask for. A surgeon showing only three-week results is showing you the part before the scar has had its say.

Cost, safety and choosing a surgeon
