A back lift, or bra-line back lift, removes a horizontal strip of excess skin and fat across the upper back, then closes the back over a flat contour. It is aimed squarely at the rolls that sit at and just below the bra line. The incision is planned so that the final scar hides under a bra strap or a swimsuit band, which is why the marking is done with you standing and, ideally, wearing the bra you actually wear. Liposuction is usually performed alongside it to blend the areas above and below the excision so the result is a smooth transition rather than a step.
Younger patients with good elasticity and genuine fullness often do very well with awake liposuction alone: the fat comes out, the skin retracts, and there is no scar. Patients whose skin has been folded in the same place for years, and patients after significant weight loss, have skin that has lost its ability to retract — take the fat out and the roll simply becomes an empty fold. In that situation liposuction alone makes the appearance worse rather than better. Your surgeon should pinch and release the roll in front of you and explain which of the two you are, and it is entirely normal for the answer to be both, in which case both are done.
Good candidates have fixed rolls along the bra line that do not change much with weight, stable weight for six months, good general health, and acceptance of a horizontal scar. Many are post-bariatric patients for whom the back is one of several areas being addressed. Others are simply people whose skin has aged out of retracting. If you are still actively losing weight, wait until it settles, because further loss will leave new laxity and you do not want this operation twice.
Under general anesthesia in an accredited facility, usually one and a half to three hours. You are marked standing. The operation is usually performed with you lying face down. Liposuction is done first where volume needs reducing, then the marked strip of skin is excised down to the fascia. The closure is layered, with the deeper layer taking the tension so the skin edges are not fighting it — that is what keeps the scar narrow. Drains are commonly used for the first several days. You wake in a compression garment, worn for around six weeks.
The back is unusual in that the discomfort is mostly about position: you cannot lie flat comfortably for the first week or two, and sleeping on your side takes some arranging. Drains come out within the first week or so. Most people return to desk work at ten to fourteen days. Reaching overhead and behind you is restricted for a few weeks, and lifting waits four to six. Swelling and firmness across the back settle over six to eight weeks. The scar goes through the usual sequence — raised and pink for three months, softening by six, pale and flat by a year — and silicone gel plus sun protection genuinely help.
A back lift is rarely the only thing on the list. It combines naturally with arm lifts, with flank liposuction, and with breast surgery, since all of them are approached at similar sessions and the areas flow into one another. Doing them together means one anesthetic, one recovery period and one set of time off work, and it usually costs less than the same operations staged separately. The limit is total operating time and safety: your surgeon should tell you where that limit falls for your case rather than agreeing to everything at once.
