What a chin implant is

A chin implant, or genioplasty with an alloplastic implant, is a solid silicone implant placed directly on the chin bone to increase projection, width, or both. It sits in a precisely made pocket on the bone, beneath the muscle and soft tissue, and is usually secured with a screw or suture so it cannot move. Implants come in a range of shapes and sizes: some add forward projection only, others extend along the jaw to widen the whole lower face. Sizing is the entire game — the difference between a balanced profile and an overdone one is a few millimetres, and it is decided at the consultation with measurements and imaging rather than by eye on the day.

Why the chin matters more than people think

The eye reads faces in proportion. In profile, the chin should sit close to a line dropped from the lower lip; when it falls behind that line, the nose reads as larger, the lips read as more prominent, and the neck reads as fuller than it is — because the angle between chin and neck is shortened. This is why a great many patients who come in asking about a nose reduction or a neck lift are shown, with imaging, that a chin implant would address more of what bothers them. It is also why the two are so often done together: balancing the nose and chin at the same time produces a result neither achieves alone.

Implant or filler

Filler in the chin is a reasonable way to test the idea, and for small deficiencies it can be enough. Its limits are real: it adds soft tissue volume rather than bony projection, it cannot achieve the same forward projection without using a great deal of product, and it wears off in nine to eighteen months, which becomes an ongoing cost. An implant provides a precise, permanent change to the underlying contour and, per year, generally costs less than maintaining filler. Many patients quite sensibly do filler first, see the effect for a year, and then decide.

How the operation is done

Under local anesthesia with sedation, or general, in an accredited facility, taking around an hour. There are two routes. Through a small incision under the chin, where the scar sits in a natural shadow, or through an incision inside the lower lip, which leaves no external scar at all but carries a slightly different risk profile. A pocket is made directly on the bone, sized to the implant so it cannot rotate. The implant is placed centrally against a marked midline — asymmetry here is visible, so this step is deliberate — and fixed. The tissue is closed in layers and a supportive dressing or tape is applied.

Recovery, honestly

Expect swelling and bruising around the chin and jaw for the first week, and a tight, stiff feeling when you speak or smile. A soft diet for several days, particularly with an intraoral incision. Most people return to work at five to seven days and look normal to others at two weeks. Numbness of the lower lip and chin is common in the first weeks — the mental nerve runs nearby and is easily bruised — and it almost always resolves, though it can take a few months. Tape or a dressing is worn for the first week. Contact sport waits six weeks. Final contour is clear at around six weeks.

What can go wrong, and how it is managed

The specific concerns with chin implants are malposition, where the implant sits off centre or too low; infection, which is uncommon but generally means the implant has to be removed and replaced later; and bone resorption, where the bone beneath a large implant remodels slightly over years — usually of no consequence but a reason not to oversize. All of these are manageable, and the implant can be removed or exchanged, which is a genuine advantage over permanent fillers or bone-cutting procedures. Sliding genioplasty, where the chin bone itself is cut and advanced, is the alternative for large deficiencies and is discussed where relevant.
