What a facelift is

A facelift repositions the deeper structures of the lower face and jawline. Under the skin there is a continuous sheet of muscle and fascia called the SMAS, and it is this layer, not the skin, that carries the shape of the face. With age it loosens and slides forward and down: the cheek fat that used to sit high drops toward the jaw, the jawline breaks into jowls, and the fold from nose to mouth deepens. A facelift releases that layer, lifts it up and back to where it sat, and secures it there. The skin is then redraped over the new foundation and the excess removed. Skin is the last thing addressed, not the first.

Why it is not a skin operation

For decades facelifts were exactly that — pull the skin, trim it, close. It works for a year or two, and then the skin stretches again, because skin is elastic and was never load-bearing. Worse, tension on the skin is what produces the giveaway signs: a mouth pulled sideways, an earlobe dragged down, a face that looks stretched over its own bones. Lifting the SMAS puts the tension where tissue can hold it and leaves the skin to lie flat. That is the whole reason a well-done modern facelift reads as someone who slept well rather than someone who had surgery.

Who makes a good candidate

The best candidates have visible jowling or midface descent, reasonable skin elasticity, a stable weight and no medical reason to avoid general anesthesia. Age matters less than the pattern of ageing; there are people at forty-eight who need it and people at sixty-five who do not. If the concern is confined to the neck, a neck lift may be all that is required. If the skin is heavily sun-damaged, a lift restores position but not surface, and resurfacing is usually discussed alongside it. Smoking is the single biggest modifiable risk, because nicotine constricts the small vessels that keep the lifted skin alive, and you will be asked to stop well before surgery.

How the operation is done

It is performed under general anesthesia in an accredited facility. Incisions follow the hairline at the temple, run down in the natural crease in front of the ear, curve around the earlobe and finish behind it, so that essentially all of the scar sits in a line or a shadow. The skin is lifted off the SMAS, the SMAS is released and repositioned, and it is fixed with sutures that hold while it heals into place. Fat can be repositioned or grafted at the same time where the face has hollowed as well as descended. The skin is then redraped, the excess is trimmed, and everything is closed in layers. Most facelifts take several hours; you go home the same day or stay overnight.

Recovery, honestly

The first three days are the worst of it: wrapped, swollen, bruised, and not seeing anyone. Sutures come out around day seven. Bruising turns yellow through the second week and can be covered with makeup by the end of it. By three to four weeks most people are back at work and out socially, still swollen in a way strangers do not notice but you do. Numbness in front of and behind the ears is normal and resolves over months. Tightness eases. The result you are actually buying appears somewhere between three and six months, once the last of the swelling has gone and the scars have faded from pink to pale. Anyone promising you a finished face at two weeks is selling something.

How long the results last

A facelift does not pause ageing; it resets the clock. You continue to age from the new starting point at your own rate. In practice most patients look meaningfully younger than they otherwise would for ten years or more, and deeper-plane techniques hold longer than superficial ones. Things that shorten it: significant weight change, heavy sun exposure, and smoking. Things that extend it: sun protection, stable weight, and good skin care. Many patients later choose a smaller secondary procedure rather than a second full lift, because there is less to move the second time.

Cost, safety and choosing a surgeon
