What Botox is and how it works

Botulinum toxin type A is a purified protein that temporarily blocks the signal between nerve and muscle. Injected in small, precise doses into a specific muscle, it reduces that muscle's ability to contract. Because dynamic lines — the ones that appear when you frown, raise your brows or smile — are creases folded into the skin by muscle movement, relaxing the muscle lets the skin lie flat and the line softens. It is not a filler and it adds nothing; it takes movement away. The effect is temporary because nerve endings regenerate, which is why treatment is repeated every three to four months.

What it treats well, and what it does not

It works reliably on lines caused by movement: horizontal forehead lines, the vertical frown lines between the brows, and crow's feet at the outer eyes. It also has established uses beyond lines — softening a gummy smile, slimming the jaw where the masseter muscle is overdeveloped, lifting the outer brow slightly, and reducing excessive sweating in the underarms. What it does not do is fill. Static lines that are visible when your face is completely still, deep folds from nose to mouth, hollow temples and thin lips are volume problems, and no amount of toxin addresses them — that is what filler, or in some cases resurfacing, is for.

Dosing, and why results differ so much

Two people can have the same product injected and get very different outcomes, and the reason is almost always dose and placement. Muscle strength varies considerably: a strong frown muscle needs more units than a weak one, and men typically need more than women. Injecting the forehead without accounting for the fact that the forehead muscle is the only thing holding the brow up is how brows get dropped. Treating one muscle group in isolation when they work in balance is how expressions end up looking odd. A good injector examines your face at rest and in motion, doses to your anatomy, and brings you back at two weeks to check.

What the appointment is like

The whole thing takes ten to twenty minutes. Your face is assessed while you frown, raise your brows and smile. The areas are cleaned, and a very fine needle is used for a series of small injections — most people describe it as a brief sting rather than pain, and numbing cream is available for anyone who wants it. There is no downtime. You will be asked to stay upright for a few hours, avoid rubbing the treated areas, and skip strenuous exercise, saunas and alcohol for the rest of the day. Small red bumps at the injection points settle within an hour, and occasional bruising resolves within a few days.

Onset, duration and maintenance

Nothing happens on the day. The effect begins at three to seven days and is complete at around two weeks, which is when your result should be reviewed and topped up if any area needs it. It then holds for roughly three to four months before movement gradually returns. Duration varies with dose, with the muscle treated, with metabolism and with how much exercise you do. Patients who treat regularly often find the interval lengthens over time, because a muscle that has been relaxed repeatedly becomes weaker. Letting it wear off completely between treatments is fine and does nothing harmful; it simply means starting from the beginning each time.

Safety, and who should not have it

Botulinum toxin has been used cosmetically for decades and has a well-established safety record when administered by a trained injector in a medical setting. Side effects are usually minor and temporary: bruising, tenderness, headache for a day or two. The two that matter are eyelid or brow droop from product diffusing where it was not intended, which is uncommon, resolves as the effect wears off, and is largely a function of placement. It should not be used during pregnancy or breastfeeding, or in people with certain neuromuscular disorders, and any previous reaction should be mentioned before treatment.

Cost, and choosing an injector
