What an arm lift is

Brachioplasty removes excess skin — and the fat attached to it — from the upper arm, then closes the arm over a tighter contour. The incision usually runs along the inner arm from the armpit toward the elbow, positioned so that it faces your body rather than the room. How far it extends depends entirely on how much skin there is: a small amount of laxity confined to the upper arm can sometimes be handled with a short scar hidden in the armpit, while significant hanging after major weight loss needs the full length. Liposuction is often done at the same time to thin the arm before the skin is closed, which gives a smoother line than either does alone.

When liposuction is enough, and when it is not

The test is simple and you can do it yourself. Pinch the skin on your upper arm and let go. If it snaps back, your problem is volume, and awake liposuction will slim the arm without a long scar. If it stays where you put it, your problem is elasticity, and removing fat will only leave the same skin with less inside it — often looking worse. Most patients under forty with heavy arms and no weight-loss history are in the first group. Most post-bariatric patients are firmly in the second. Who makes a good candidate

Good candidates have significant loose upper arm skin, a stable weight for at least six months, general good health, and a clear understanding of the scar. If you are still losing weight, wait — this operation is worth doing once, on the body you intend to keep. Smoking materially raises the risk of wound-healing problems along a long incision under tension and must stop well before surgery. Patients who cannot accept a visible scar on the inner arm should not have this operation, and a good surgeon will say so rather than talk them into it.

How the operation is done

Under general anesthesia in an accredited facility, usually taking two to three hours for both arms. Markings are made with you standing and the arm raised, because skin lies differently when you are lying down. Liposuction is often performed first to reduce volume and free the skin. The marked skin is then excised, the deeper fascial layer is closed to take the tension off the skin, and the incision is closed in layers with fine sutures. Drains are sometimes used. You wake in compression sleeves, which you will wear for several weeks; they are as much about controlling swelling as anything else.

Recovery, honestly

Arms are awkward for the first week: swollen, tight, and difficult to raise above shoulder height. You will need help with anything overhead. Most people are off strong pain relief within a few days and back to desk work at one to two weeks. Sutures or drains come out within the first week or two. Lifting anything heavy waits four to six weeks, and full gym work six. Swelling settles over two to three months. The scar is the slow part: raised and pink for the first three months, softening over six, and pale and flat by twelve to eighteen months. Silicone sheeting or gel from around three weeks, and sun protection, genuinely make a difference.

Living with the scar

This is the part that deserves plain talk. The scar runs the length of the inner arm and it is permanent. Placed correctly, it faces your side and is invisible from the front, in most clothing, and in almost every photograph. Patients who lost a great deal of weight overwhelmingly report they would do it again, because the hanging skin was a daily reminder and the scar is not. Patients who had mild laxity and expected an invisible line are the ones who regret it. Which of those you are is worth being honest about at the consultation, and there is no wrong answer.
