What rib remodeling is

Rib remodeling — sometimes called rib repositioning — narrows the lower ribcage by carefully repositioning the floating ribs inward. The lower ribs are not fixed to the sternum at the front, which is what makes them movable. Through small incisions on the back the ribs are accessed, controlled and repositioned so that the widest point of the ribcage sits narrower than it did, and they heal in that new position. It is almost always combined with liposuction of the waist and flanks, because the aim is a continuous line from ribs to hips rather than a narrower skeleton under unchanged soft tissue.

Why liposuction has a ceiling

Liposuction removes fat that sits outside the abdominal wall. It cannot remove fat inside the abdomen, it cannot narrow the muscle wall, and it certainly cannot narrow bone. That means every waist has a floor set by the frame underneath it. For most patients that floor is well below where they currently are, which is why liposuction produces such dramatic results in the majority. For a lean, trained patient who has already had their waist contoured, the remaining width is structural, and no amount of further liposuction will change it — attempting it only thins the tissue until the result looks unwell. Rib remodeling exists specifically for the small group who have reached that floor.

Who makes a good candidate

Good candidates are at a low, stable body fat, are in good general health, have often already had waist liposuction, and want a change in proportion rather than in weight. They are people who understand that this is a bigger operation than the one they had before, with a longer and more uncomfortable recovery. Patients who have not yet had liposuction, or who are not near their stable weight, should do the simpler thing first — the result may well be all they wanted. Anyone with a significant respiratory condition needs careful assessment, since the ribcage is involved in breathing.

How the operation is done

Under general anesthesia in an accredited facility. Imaging is reviewed beforehand so the surgeon knows the anatomy they are working with. Small incisions are made on the back, well away from the front, and the lower ribs are accessed through the muscle. The ribs are then repositioned inward under direct control and secured so they heal in place. Liposuction of the waist, flanks and often the upper abdomen is performed at the same time to complete the line, and fat is sometimes transferred to the hips to strengthen the ratio further. A compression garment goes on before you wake.

Recovery, honestly

This recovery is more demanding than liposuction and should be planned for. The first week involves significant soreness with breathing, coughing and moving in bed, and you will be given specific instructions on breathing exercises because keeping the lungs expanded matters. Most people take two to three weeks off work and longer for anything physical. Compression is worn for six to eight weeks. Impact exercise waits eight to twelve weeks, and heavy lifting longer, because the ribs need to heal in position. Swelling in the waist is substantial and takes three to six months to fully resolve, so measurements taken early will disappoint you.

What it can and cannot do

It narrows the lower ribcage and, combined with liposuction and often fat transfer to the hips, it changes the waist-to-hip ratio in a way nothing else can. It does not make you thinner, it does not treat the abdominal wall, and it does not compensate for weight that is still moving. It also does not turn any body into any other body: the result is bounded by your own anatomy, and a surgeon who cannot describe those bounds specifically for you at the consultation is not the right surgeon for this operation.
