What pectoral implants are

Pectoral implants are solid silicone implants placed underneath the pectoralis major muscle to add size, projection and width to the chest. Unlike breast implants they are not filled with gel or saline; they are soft solid silicone that cannot rupture or leak. They come in different shapes and sizes, and the choice is made against your frame, your existing muscle bulk and what you actually want the chest to look like. Placed under the muscle, the implant is covered by muscle in exactly the same way your own tissue is, which is what makes the result read as a chest rather than as an object under the skin.

Implants or fat transfer

The honest comparison comes down to how lean you are and what you want the chest to feel like. Fat transfer takes fat from elsewhere by liposuction and grafts it to the chest; it feels entirely natural, it improves the donor area at the same time, and it is limited by how much fat you have and by the proportion that survives — typically a good deal less than what was put in. Very lean patients often do not have enough to harvest. Implants provide substantial, predictable projection regardless of body fat, but they feel firm and they are a device. Many patients ultimately combine the two, using an implant for structure and fat to smooth the edges.

Who makes a good candidate

Good candidates are in good health, at a stable weight, and have trained consistently without getting the chest development they want — the key word being consistently, because an implant is not a substitute for training that has not happened. Patients with congenital differences such as Poland syndrome, where the pectoral muscle is partly or entirely absent on one side, are among the most transformed by this operation. Patients who compete in sports where a chest implant would be a problem should discuss that specifically. Realistic expectations about how the chest feels are essential.

How the operation is done

Under general anesthesia in an accredited facility, usually taking one and a half to two hours. The incision is made in the armpit, along a natural crease, so there is no scar on the chest itself. A pocket is created beneath the pectoralis major muscle, sized precisely to the implant so it cannot rotate or displace, and the implant is inserted and positioned. Both sides are compared throughout, and where the chest started asymmetric, different implants may be used. Drains are sometimes placed. You wake in a compression garment worn across the chest.

Recovery, honestly

The first week is the demanding part: the muscle has been lifted off the chest wall and it protests, particularly when you move your arms. Reaching, pushing and lifting overhead are all restricted, and you will want help for a few days. Most people return to desk work at one to two weeks. Compression is worn for around six weeks. Upper body training is off the table for six to eight weeks, and pressing movements come back last — usually eight to twelve weeks — because they act directly on the muscle covering the implant. Swelling settles over six weeks; the implants settle into their final position over two to three months.

What it does and does not do

It adds size, projection and width to the chest. It does not build muscle, it does not improve definition on its own, and it will not show at all under a layer of fat — patients carrying weight over the chest are usually better served by liposuction first, or by both. It also does not remove the need to train: an implant sitting under a muscle that is not being maintained looks less convincing over the years. For men whose chest fullness is glandular rather than muscular, the diagnosis is gynecomastia and the answer is a different operation entirely.
