What a canthopexy is

At the outer corner of each eye there is a tendon — the lateral canthal tendon — anchoring the eyelids to the bony rim of the orbit. Its position determines the tilt of the eye. A canthopexy tightens and repositions that tendon slightly higher on the rim, which lifts the outer corner and gives the eye a more almond, upswept shape. The related operation, canthoplasty, detaches and reattaches the tendon rather than tightening it in place, and is used where there is more laxity or where a previous surgery has pulled the lid down. Both are measured in millimetres, and both are frequently performed alongside eyelid surgery rather than alone.

Canthopexy and the thread lift confusion

A great deal of what is marketed as a fox eye or cat eye lift is a thread lift: dissolvable barbed threads passed under the skin of the temple and tensioned to pull the outer brow and corner upward. It is not the same procedure. Threads produce a temporary effect lasting months rather than years, can produce visible dimpling or migration, and do nothing to the tendon that actually sets the eye's tilt. A surgical canthopexy addresses the anatomy directly and lasts. If you are choosing between them, the honest comparison is a small permanent change against a larger temporary one.

Who makes a good candidate

Good candidates have an outer corner that sits level with or below the inner corner, and want a subtle upward tilt. It is particularly useful in patients with a lax lower lid, in whom lower eyelid surgery without canthal support risks the lid pulling away from the eye afterwards — in that setting it is less a cosmetic addition than a structural necessity. Patients hoping for a dramatic change in eye shape are not good candidates: the tendon can only be moved a few millimetres before the eye looks unnatural and the lid stops sitting properly against the globe.

How the procedure is done

Usually under local anesthesia with sedation, in an accredited facility, taking thirty to sixty minutes on its own. A small incision is made at the outer corner, frequently hidden in an existing crease or combined with the incision used for eyelid surgery. The canthal tendon is identified, tightened, and secured to the periosteum at the desired height on the orbital rim. The height is set slightly above the intended final position, because the corner relaxes as it heals. The incision is closed with fine sutures. When combined with lower eyelid surgery it adds relatively little to the operating time or the recovery.

Recovery, honestly

Expect bruising and swelling around the outer corner for the first week to ten days, and a tight or slightly pulling sensation when you blink. Sutures come out at five to seven days. Most people are presentable with a little makeup at two weeks. The corner looks over-lifted at first and this settles over four to six weeks — patients who are not warned about this find the first fortnight alarming, and there is no need to be. Contact lenses wait around two weeks, exercise three. Final position is clear at six to eight weeks.

Limits, and when it is the wrong procedure

A canthopexy changes the tilt of the eye. It does not remove hooded upper lid skin, which is eyelid surgery, and it does not raise a heavy brow, which is a brow lift. Patients who bring in photographs of a dramatically elongated eye are usually looking at a combination of makeup, brow position and, frequently, digital editing. A good consultation will separate what can be achieved surgically from what cannot, and will say plainly if the change you want is not available in this anatomy — which is a better outcome than an over-tightened corner that pulls the lid away from the eye.

Cost, safety and choosing a surgeon
