Narrows the waist by reshaping the lowest ribs, for a change liposuction cannot make.
The floating ribs are reshaped through small incisions in the back, allowing the waist to narrow. It is done alongside FlexSculpt so the soft tissue follows the new frame.
Imaging and examination establish whether your waist is limited by fat, by muscle, or genuinely by bone.
The floating ribs are carefully repositioned inward through small incisions on the back.
Liposuction of the waist and flanks is nearly always done alongside, since the aim is the whole line.
A wide lower ribcage — the waist that stays broad even at a low body weight, because the limit is bone rather than fat.
You are lean, you train, and the waist still measures wide.
Or your surgeon has told you the remaining width is skeletal.
You want the ratio changed, not just the fat reduced.
This is a specialised operation and it deserves a specialised conversation.
How rib remodeling compares with awake liposuction.
| What it changes | The position of the lowest ribs | Fat over the waist and flanks |
|---|---|---|
| Ceiling on the result | Set by your skeleton, and moved | Set by your skeleton, and fixed |
| Anesthesia | General | Local, and you are awake |
| Downtime | Three to four weeks | Days |
| Best for | A waist that stays wide however lean you get | Fullness over the waist with a narrow frame underneath |
Here is how it goes, step by step.
Sit down with your surgeon in Atlanta Walton St., go over your goals and your medical history, and confirm rib remodeling is the right operation.
What is being done, where the incisions go, what it will cost, and what result is realistic for your anatomy.
Bloodwork and medical clearance, what to stop taking, and how to set your home up for the first week.
You arrive, you are marked and prepped, and the operation is performed by a licensed U.S. surgeon in an accredited facility.
Follow-ups, drains or sutures out where relevant, and clear instructions you can actually follow.
Swelling resolves over months, not days. The final result is judged at the end of that, not the week after surgery.
Recovery is a sequence, not a date. This is the shape of it.
Compression, restricted twisting and lifting. Deep breaths are uncomfortable.
Desk work returns. No core exercise.
Gradual return to training.
Final contour once swelling has fully gone.
Rib remodeling changes the frame rather than what sits on it. It is a bigger undertaking than liposuction, with a longer recovery, and the result takes several months to appear fully.
Measurable, because the frame itself has changed and not only what covers it.
The ratio between waist and hips changes in a way liposuction alone cannot achieve.
Bone that has healed in its new position stays there.
Swelling in this area is substantial and takes months to resolve fully.
Every procedure is performed by a licensed U.S. surgeon in an accredited facility. Goals is a management company; the medical practice and the surgeon responsible in your state are named in our terms.

It is a considered operation done by surgeons who perform it regularly, in an accredited facility. It is also not for everyone, and a consultation is where that is decided honestly.
Anyone whose waist is limited by fat rather than frame. FlexSculpt is the answer there and it is a far smaller operation.
They are controlled and repositioned, not shattered. They then heal in the new position, which is why the recovery involves restrictions on lifting and impact while that healing happens.
Breathing is sore for the first weeks and you will be given exercises to keep the lungs fully expanded. Long-term lung function is not the aim of the operation to change, and patients with existing respiratory conditions need careful assessment first.
Usually, yes. If your waist has not yet been contoured, the simpler operation may give you everything you were after, and it is the honest place to start.
This page is about rib remodeling at Goals in Atlanta. The clinic is at 83 Walton St NW, Atlanta, GA 30303, and consultations there are free and carry no obligation. Surgeons operating here include Dr. Alla Zemlyak, MD, Dr. Patrick Narh-Martey, MD. Everything below — what the operation does, who it suits, what recovery involves and what it costs — is the same wherever you have it done, because the anatomy is. What changes from clinic to clinic is who you meet, how far you travel, and how easily you can get back for follow-ups, and those are the things worth deciding on.
Our Atlanta clinic is at 83 Walton St NW, Atlanta, GA 30303, and it is where your consultation, your procedure and your follow-ups all happen — you are not consulted in one place and operated on in another. Consultations are free, run in English or Spanish, and end with a written quote rather than a range. Patients travelling in for rib remodeling should plan to stay locally for the first night or two rather than fly home the same day, and to be back at the clinic for their early follow-up. We also operate in Atlanta, so if another address in Georgia is an easier drive, the same procedure is available there at the same price. Whichever clinic you choose, the surgeon is licensed in that state and the facility is accredited, and the practice responsible for your care is named in our terms rather than left vague.
Rib remodeling — sometimes called rib repositioning — narrows the lower ribcage by carefully repositioning the floating ribs inward. The lower ribs are not fixed to the sternum at the front, which is what makes them movable. Through small incisions on the back the ribs are accessed, controlled and repositioned so that the widest point of the ribcage sits narrower than it did, and they heal in that new position. It is almost always combined with liposuction of the waist and flanks, because the aim is a continuous line from ribs to hips rather than a narrower skeleton under unchanged soft tissue.
Liposuction removes fat that sits outside the abdominal wall. It cannot remove fat inside the abdomen, it cannot narrow the muscle wall, and it certainly cannot narrow bone. That means every waist has a floor set by the frame underneath it. For most patients that floor is well below where they currently are, which is why liposuction produces such dramatic results in the majority. For a lean, trained patient who has already had their waist contoured, the remaining width is structural, and no amount of further liposuction will change it — attempting it only thins the tissue until the result looks unwell. Rib remodeling exists specifically for the small group who have reached that floor.
Good candidates are at a low, stable body fat, are in good general health, have often already had waist liposuction, and want a change in proportion rather than in weight. They are people who understand that this is a bigger operation than the one they had before, with a longer and more uncomfortable recovery. Patients who have not yet had liposuction, or who are not near their stable weight, should do the simpler thing first — the result may well be all they wanted. Anyone with a significant respiratory condition needs careful assessment, since the ribcage is involved in breathing.
Under general anesthesia in an accredited facility. Imaging is reviewed beforehand so the surgeon knows the anatomy they are working with. Small incisions are made on the back, well away from the front, and the lower ribs are accessed through the muscle. The ribs are then repositioned inward under direct control and secured so they heal in place. Liposuction of the waist, flanks and often the upper abdomen is performed at the same time to complete the line, and fat is sometimes transferred to the hips to strengthen the ratio further. A compression garment goes on before you wake.
This recovery is more demanding than liposuction and should be planned for. The first week involves significant soreness with breathing, coughing and moving in bed, and you will be given specific instructions on breathing exercises because keeping the lungs expanded matters. Most people take two to three weeks off work and longer for anything physical. Compression is worn for six to eight weeks. Impact exercise waits eight to twelve weeks, and heavy lifting longer, because the ribs need to heal in position. Swelling in the waist is substantial and takes three to six months to fully resolve, so measurements taken early will disappoint you.
It narrows the lower ribcage and, combined with liposuction and often fat transfer to the hips, it changes the waist-to-hip ratio in a way nothing else can. It does not make you thinner, it does not treat the abdominal wall, and it does not compensate for weight that is still moving. It also does not turn any body into any other body: the result is bounded by your own anatomy, and a surgeon who cannot describe those bounds specifically for you at the consultation is not the right surgeon for this operation.
Rib remodeling at Goals starts at $10,000, confirmed at consultation, with a $500 deposit to reserve your date. Liposuction and fat transfer performed at the same time are quoted with it, so you know the total before you book. This is a specialised operation and the risks are correspondingly specific: pain with breathing, pneumothorax, prolonged discomfort, asymmetry, and a longer recovery than patients expect. Ask directly how many of these your surgeon has performed, ask what imaging they use, and ask to see results at six months rather than at six weeks — early photographs of this operation are swollen and prove nothing.
